If your account was registered in the name of an individual person, you will need to provide a color copy of that individual’s Drivers License or State ID.
If the account had a company name listed during registration, you will need to submit a copy of one of the following documents as proof of ownership.
Business License
Certificate of Association
501(c)(3)
DBA/ Fictitious Name Documentation
Individual/Company Name
Street Address matching the address on the account
